Help needed for a complete novice

I would really appreciate some help with a couple of questions I have about my NVQ.
I have just started a level 3 retail course and have fallen at the first hurdle. I am needing to know how I should be completing the paperwork.
For my first section, how to work effectively in your organisation, I have a sheet with a long list of 'questions' on it, with each 'question' having 6 boxes next to it. Do I have to give a piece of evidence for each question and why are there 6 boxes after each one? Can anyone explain what is required to complete this?
I also have a sheet that asks for observation/assessment activities. This has 6 large boxes on it, what do I have to do in order to complete this?
If anyone can shed light on this would be greatly appreciated!

Normally, when you start an NVQ you should have a person working with you as your assessor. This person will help you plan your assessment and instruct you on how to complete any paperwork.

Unfortunately my assessor is

Unfortunately my assessor is away and I cannot for the life of me remeber how to fill in the paperwork hence my post.
I would love some help with this so that I can crack on with my work load while he is away.

Centres will often have different paperwork. If you contact your centre they should be able to advise you.
