It would depend very much upon the reason for the cancellation surely? Some things just can't be helped, alternatively if it part of an overall pattern of behaviour then it might be classed as "unreliable" However, be very careful about the word that you use to describe a learner...............it might be better to address the issue as part of a review?
Just to add to AJC's answer. In competence based assessment, reliability is a term used to judge evidence not the candidate. Since the observation did not take place then this is not relevant in this case.
